Sample Submission

The Pharmaceutical Analysis Request Form link provides you with an electronic copy of the form that can be downloaded to your computer, completed and then e-mailed back to your project manager.

In order to make submitting samples to Eurofins Lancaster Laboratories a streamlined process and to get the critical information we need to provide you with the highest quality laboratory services, we’ve created a guide to help you complete our Pharmaceutical Analysis Request Form.

The Sample Submission Guide will take you through each section of the form, noting what each section is for, and how to complete it. This guide also includes an example of a Completed Pharmaceutical Analysis Request Form will give a general overview of how a completed form might look

If you are using our Raw Materials Express Program, please use the Raw Materials Express Pharmaceutical Analysis Request Form when submitting samples.

To ensure the most efficient sample entry, when submitting samples we ask that you:

• Use one analysis request form per quote and sample material.
• Include a hard copy of the completed analysis request form with your sample submission.
• Send a PDF of the completed analysis request form to your project manager prior to shipping your samples. By notifying your project manager before samples are shipped, he/she can expedite the testing process by having all the paperwork prepared in advance.
